IMPORTANT INFORMATION TO PATIENTS OF
DR SONNATHI & VEERAPPAN and
DR BOYAPATI’S SURGERY
TUNSTALL PRIMARY CARE CENTRE
On 1 April 2017, Drs Sonnathi & Veerappan’s Surgery will merge with Dr Boyapati’s surgery to become a single practice at the Tunstall Primary Care Centre site in Stoke on Trent. Dr Bose will be retiring from practice at the same time.
Although the surgery name will change, the practice will continue to provide the same services that are currently provided. The new surgery will be named Tunstall Primary Care.
The branch surgery at Whitfield Valley Centre in Fegg Hayes will also close from 31 March 2017. All services from 1 April will be provided from the Tunstall Primary Care Centre only.
This merger will ensure that we continue to provide high quality GP services. It will also enable us to offer a wider range of services for our patients.
You do not need to take any action - your health records will automatically transfer over to Tunstall Primary Care on 1 April 2017.
You can use either telephone number to contact the practice for medical care and appointments: 0300 123 0978 or 0300 790 0230.
Should you wish, you do have the option to register with an alternative GP practice. You can find your nearest GP surgeries by putting your postcode into the NHS Choices website at www.nhs.uk
If you have any queries please speak to one of the practice staff.
Welcome to our website where we hope you will find enhanced convenience added to first class healthcare. Find out when we are open and how to see a member of the team. We’ve made life a little easier with practice updates, news on the latest health issues and even access to repeat medication all at your fingertips.
Named GP for all registered Patient's
From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a named accountable GP.
This is purely an administrative exercise in order that patients can have a named responsible GP should they require them.
For convenience your named accountable GP will be the named doctor you are registered with.
If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.
Please note: there is no need to telephone the practice for this information.
If you need input from a variety of health professionals, then you can discuss these with the named GP if you wish. Alternatively you can discuss these with any of the other doctors.
Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
